The MHA has updated its analysis of Medicaid and Medicare enrollment based on September 2026 data. The analysis includes program enrollment as a percentage of each county’s total population and the split between fee-for-service (FFS) and managed care organization (MCO). Nearly half of Michigan’s total population is enrolled in Medicaid (24%) or Medicare (23%).
Roughly two-thirds of Michigan’s nearly 2.4 million Medicaid beneficiaries are enrolled in one of nine MCOs. Nearly 650,000 beneficiaries are enrolled in the Healthy Michigan Plan, which will be subject to Medicaid eligibility changes including work requirements that are set to go into effect Jan. 1, 2027. The MHA continues working with the Michigan Department of Health and Human Services and other stakeholders to minimize the impact of these changes that were enacted last year as part of the federal bill H.R. 1.
Total Medicare enrollment is 2.3 million, with 64% of beneficiaries enrolled in a Medicaid Advantage (MA) plan. MA enrollment by county ranges from 45% to 79%. Only three counties have less than 50% of total Medicare enrollment in MA plans, while 75 counties have 55% or more of their Medicare population enrolled in an MA plan, as highlighted below.

September enrollment is spread across 46 MA plans; at least five plans are available in each county, while several Michigan counties have up to 30 plans that cover beneficiaries.
