Two Training Options Available for Community Health Worker Sustainability Program

The Practice Transformation Institute will offer Community Health Workers (CHW) Sustainability: Advancing the Profession, a live virtual training focused on integrating and sustaining CHWs within healthcare, social services and community-based organizations.

The two-part program will address community health worker billing services, Michigan payer requirements, HCPCS codes, social determinants of health Z-codes, organizational workflows and documentation requirements. The training is designed for CHWs and professionals who regularly work with CHWs or want to better understand their role within care teams.

Two training options are available, and registration is required:

  • Sept. 24 and Oct. 1, from 8 a.m. to noon
  • Nov. 19 and 20, from 8 a.m. to noon

The cost is $199 per person. Participants must attend both sessions in their selected training option to receive continuing education units (CEUs). The program offers 0.8 International Accreditors for Continuing Education and Training CEUs and is approved for eight Michigan Community Health Worker Alliance CEUs.

Members with questions may contact Lenise Freeman at the MHA.
