The Michigan Department of Health and Human Services recently notified stakeholders that some sexual assault evidence kits with barcode numbers beginning with “MI26” are missing the Authorization for Release to Law Enforcement or Storage Without Release to Law Enforcement form due to a printing error.
The MDHHS recommends printing copies of the Authorization for Release to Law Enforcement or Storage Without Release to Law Enforcement form and storing the forms with kits to help ensure page 10 is available if an original form is missing.
Hospitals are encouraged to share the information with emergency department directors, sexual assault program directors and team members who oversee or perform sexual assault forensic examinations. If a kit is missing the Authorization for Release to Law Enforcement or Storage Without Release to Law Enforcement form, teams should email MSP-SAEK@Michigan.gov with the following information:
