Create Sustainable Systems-based Improvements

The MHA Keystone Center Patient Safety Organization (PSO) hosted a Root Cause Analysis & Action (RCA²) workshop tailored to healthcare professionals who wish to increase their knowledge of the Root Cause Analysis (RCA) process.  The training sessions assisted in the implementation of sustainable systems-based improvements within organizations.

The event was led by two members of the core group that developed the RCA² process: Jim Bagian, MD, PE, director, Center for Healthcare Engineering and Patient Safety, and professor, University of Michigan; and Joe DeRosier, PE, CSP, program manager, Center for Healthcare Engineering and Patient Safety. The training sessions were based on the National Patient Safety Foundation’s (NPSF) RCA²: Improving Root Cause Analyses and Actions to Prevent Harm guidelines.
