MHA Keystone Center Leadership Corner: August 2017

The Leadership Corner features monthly updates from the Michigan Health & Hospital Association (MHA) leadership team. The updates will provide new insights to patient safety and quality as well as information obtained from healthcare workshops and conferences across the country.

Gary RothGary Roth, DO, MBA, FACOS, FCCM, FACS, chief medical officer, Michigan Health & Hospital Association (MHA) shares an update on the status of the MHA High Reliability Organization (HRO) journey.

The term ‘journey’ has been coined by the MHA Keystone Center to describe its partnership between the Joint Commission Center for Transforming Healthcare (CTH) and one hundred MHA member hospitals.  Journey suggests the quest is ongoing as eliminating patient harm has no end.

Terms such as, ‘initiative,’ ‘collaborative,’ and ‘project,’ have purposely been avoided as HRO is a journey about transforming daily work to support the ongoing pursuit of zero preventable harm to patients and caregivers. 

Patients expect to be healed, not harmed, and staff expect to go home injury-free each day.

Through the MHA partnership with CTH, Michigan hospitals have been providing support in adopting principles of high reliability.  They are participating in many learning opportunities as well as sharing experiences and best practices.  All Michigan Hospital Improvement Innovation Network (HIIN) hospitals are designated as Tier-1.  A subset of volunteers are also engaged in Tier-2 of the process. Tier-1 hospitals are experiencing the fundamentals of high reliability while the Tier-2 are taking a ‘deeper dive’ towards becoming a HRO.

Below is a brief overview of the two tiers:


  • HRO participating hospitals include all Michigan hospitals that are within the MHA Keystone Center’s Hospital Improvement Innovation Network (HIIN)
  • 100+ hospitals are classified as Tier-1

These 100+ hospitals:

  • Have completed the ORO 2.0® High Reliability Organization Assessment and action plan process
  • Participate in monthly content webinars
  • Participate in quarterly cohort call(s) of choice (e.g. Leadership, Safety Culture or Process Improvement)
  • Attend annual MHA Keystone Center HRO workshops


  • Hospitals are subset of Tier-1 and are involved in all the opportunities within Tier-1
  • Participate in face-to-face site visits (Two per year)
  • Participate in monthly topic specific cohort calls (i.e. transparency)
  • Attend an annual Tier-2 workshop

I hope this information provided some clarity regarding the MHA Keystone Center’s statewide journey towards making Michigan hospitals HROs, as well as eliminated any uncertainty and/or confusion about the process.
