Explore Proposed Changes to CMS Discharge Planning Standards

The Centers for Medicare & Medicaid Services (CMS) recently issued a proposed rule with revisions to discharge planning requirements that hospitals, including critical access hospitals, must meet to participate in the Medicare and Medicaid programs. The proposed changes would modernize discharge planning requirements to help improve patient quality-of-care outcomes; reduce avoidable complications, adverse events and readmissions; and improve patient engagement efforts.

The MHA Health Foundation webinar “CMS Proposed Changes to the Discharge Planning Standards and IMPACT Act” will outline effective ways to explain discharge instructions to patients, determine the specific information that must be sent to primary care practitioners within 48 hours of patient discharge, and much more.

The webinar is scheduled from 10 a.m. to noon, April 17, and MHA members can register for a connection fee of $195. Members with questions should contact Erica Leyko at the MHA.
