Webinar Series Examines Readmissions

Readmission following hospitalization is common, expensive and frequently preventable. Conversely, evidence has shown that unplanned readmissions are often associated with lower quality of care. Thus, the Centers for Medicare & Medicaid Services Partnership for Patients Hospital Innovation Improvement Network (HIIN) has made reducing readmissions a top priority.

As part of this effort, the Great Lakes Partnership for Patients (GLPP) HIIN is hosting a four-part webinar series called Reducing Hospital Readmissions. Registration is free and open to all GLPP HIIN members.

The Reducing Hospital Readmissions webinar series is comprised of four separate webinars, each lasting an hour. The first installment was held Jan. 17 and focused on Transitional Care Management, which are services designed to assist in preventing readmission to an inpatient status.  

Upcoming webinar topics will include sepsis and the patient-centered approach and how these issues correlate to readmissions. Registration is required to participate in the webinars.
