RCA² are tailored to healthcare professionals who wish to increase their knowledge of the Root Cause Analysis (RCA) process. The training sessions are based off the National Patient Safety Foundation’s RCA²: Improving Root Cause Analyses and Actions to Prevent Harm guide, and will assist in the implementation of sustainable systems-based improvements within organizations. Participants will be given an introduction to RCA² elements and learn how to improve their own RCA process.

The event will be led by two members of the core working group that developed the RCA² process: Jim Bagian, MD, PE, director, Center for Healthcare Engineering and Patient Safety; professor, University of Michigan, and Joe DeRosier, PE, CSP, program manager, Center for Healthcare Engineering and Patient Safety.
